How to amend contact information after submission of application for HOS 2025 / WSM 2025 / GSH2025?

Applicant could amend contact information (including address, telephone number, email address (applicable to online application only)) by writing or via a designated amendment form, and submit to HOS / GSH / WSM Unit by post / by hand (Address: Podium Level 1, Hong Kong Housing Authority Customer Service Centre, 3 Wang Tau Hom South Road, Kowloon). 

【Amendment of Application Information Form】

• Green Form / White Form
• the form can be downloaded in HOS 2025 / WSM 2025 / GSH 2025 website during application period 
• the form is applicable for application period only and must submit on or before application deadline (i.e. 20 May 2026) 

Note: All applications are subject to approval from the Housing Department. The Housing Department may require applicants to submit relevant supporting documents and undertakings.
